Tim Follin--the soundtrack to the somewhat obscure SNES game Plok, from 1993.  He's got some clips of it up on his website.

Absolutely fantastic stuff.  The guy was a master of squeezing every last bit out of the old 8-bit and 16-bit sound chips.
